If a ladder is on a slippery surface, at an incorrect angle or is in poor condition then it is an accident waiting to happen and there is usually no warning! The bodyweight of the user will create a downward force and because the ladder is at an angle to the wall, that force is not only downward but also outward. If the ladder is going to slip, it will therefore be the user's own weight which will make it do so.
